12/29/2023 EnchantaVaughn Approved QMHPA greeted Ms. Cosby upon arrival to the home setting. Upon arrival to the home setting, Ms. Cosby appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Cosby verbalized to QMHPA that her morning was going well. QMHPA informed Ms. Cosby that this session was going to focus on reducing her depressive symptoms and learned techniques to assist with the depressive symptoms. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Cosby current thoughts, feelings and emotions. QMHPA engaged Ms. Cosby in a discussion surrounding her medication management. QMHPA engaged Ms. Cosby in a discussion about being able to cope with depression in an appropriate behavior and not allowing situations to cause her to become angry. QMHPA engaged Ms. Cosby in a skills building activity. QMHPA explained to Ms. Cosby that the purpose of this activity was to assist her with relaxation is a calming technique and will clear her mind of any negative thoughts. QMHPA explained to Ms. Cosby about being able to maintain a healthy relationship with the people around her, including family and friends. QMHPA explained to Ms. Cosby that by her eliminating some the people that are in her life with decrease some of her symptoms of depression. Ms. Cosby appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Ms. Cosby said she scheduled an appointment to see the doctor about her throat but did not make it. Ms. Ms. Cosby expressed to QMHPA that she is very depressed with everything she is currently going through as it relates to her having to deal with her boyfriend as well as her place employment. Ms. Cosby indicated to QMHPA that when it comes to her attempting to communicating with him, he always have something to say trying to cover up the roommates behavior, so because it is not her home, she tries to just keep her mouth closed and continue to do what she needs to do in the home.
